Position Overview
Health Link Home Health and Hospice seeks a compassionate Hospice Chaplain and Bereavement Coordinator to provide spiritual support and emotional guidance to patients and families during end-of-life transitions in the San Francisco Bay Area.
Key Responsibilities
- Provide spiritual guidance and emotional support to patients and families from diverse backgrounds
- Offer comprehensive bereavement counseling before and after a patient's passing
- Collaborate with interdisciplinary hospice team to deliver holistic, patient-centered care
- Help individuals and families navigate grief, loss, and end-of-life transitions with compassion
- Work independently in the field while maintaining close communication with the hospice team
Required Qualifications
- Degree or certification from an accredited seminary (BD, MDiv, or equivalent) or appropriate certification in hospital/pastoral ministry
- Experience supporting end-of-life care, bereavement, or hospice patients preferred
- Demonstrated ability to provide care in an interfaith setting
- Strong knowledge of grief counseling, bereavement support, and hospice philosophy
- Excellent team-oriented communication and collaboration skills
