Hospice Chaplain/ Bereavement Coordinator

Position Overview

Health Link Home Health and Hospice seeks a compassionate Hospice Chaplain and Bereavement Coordinator to provide spiritual support and emotional guidance to patients and families during end-of-life transitions in the San Francisco Bay Area.

Key Responsibilities

• Provide spiritual guidance and emotional support to patients and families from diverse backgrounds
• Offer comprehensive bereavement counseling before and after a patient's passing
• Collaborate with interdisciplinary hospice team to deliver holistic, patient-centered care
• Help individuals and families navigate grief, loss, and end-of-life transitions with compassion
• Work independently in the field while maintaining close communication with the hospice team

Required Qualifications

• Degree or certification from an accredited seminary (BD, MDiv, or equivalent) or appropriate certification in hospital/pastoral ministry
• Experience supporting end-of-life care, bereavement, or hospice patients preferred
• Demonstrated ability to provide care in an interfaith setting
• Strong knowledge of grief counseling, bereavement support, and hospice philosophy
• Excellent team-oriented communication and collaboration skills
