Hospice Social Worker (MSW or LCSW) for Christian provider

Position Overview

As a Christian hospice social worker, you will provide compassionate psychosocial support to patients and families during their end-of-life journey, implementing critical medical social work services with professional excellence and spiritual care.

Key Responsibilities

• Conduct comprehensive psychosocial assessments of patients and families, evaluating dynamics, caregiver abilities, communication patterns, and potential risks
• Participate in developing individualized patient care plans with the interdisciplinary team, focusing on holistic patient needs
• Provide counseling and crisis intervention for patients and families experiencing stress related to terminal illness
• Educate patients and families on advanced directives, funeral planning, and healthcare decision-making
• Maintain detailed clinical records and ensure ethical business and clinical standards are consistently met
• Assess and address cultural considerations impacting patient care and family support
• Offer information and referral services for practical and community resource needs

Required Qualifications

• Master's degree in Social Work (MSW) from Council on Social Work Education-accredited program
• Licensed MSW or LCSW in the State of South Carolina
• Minimum one year healthcare experience, with hospice experience preferred
• Strong understanding of hospice philosophy and death/dying issues
• Demonstrated expertise in grief counseling and loss support
• Excellent verbal and written communication skills
• Valid driver's license with reliable, insured transportation
• Proficiency with information systems and applicable software
