Position Overview
As a Christian hospice social worker, you will provide compassionate psychosocial support to patients and families during their end-of-life journey, implementing critical medical social work services with professional excellence and spiritual care.
Key Responsibilities
- Conduct comprehensive psychosocial assessments of patients and families, evaluating dynamics, caregiver abilities, communication patterns, and potential risks
- Participate in developing individualized patient care plans with the interdisciplinary team, focusing on holistic patient needs
- Provide counseling and crisis intervention for patients and families experiencing stress related to terminal illness
- Educate patients and families on advanced directives, funeral planning, and healthcare decision-making
- Maintain detailed clinical records and ensure ethical business and clinical standards are consistently met
- Assess and address cultural considerations impacting patient care and family support
- Offer information and referral services for practical and community resource needs
Required Qualifications
- Master's degree in Social Work (MSW) from Council on Social Work Education-accredited program
- Licensed MSW or LCSW in the State of South Carolina
- Minimum one year healthcare experience, with hospice experience preferred
- Strong understanding of hospice philosophy and death/dying issues
- Demonstrated expertise in grief counseling and loss support
- Excellent verbal and written communication skills
- Valid driver's license with reliable, insured transportation
- Proficiency with information systems and applicable software
