Position Overview
Hospice Chaplains serve a sacred role in providing compassionate spiritual support to patients and families during their end-of-life journey, offering deep listening, comfort, and meaningful spiritual care across diverse belief systems.
Key Responsibilities
- Provide comprehensive spiritual care support to patients, family members, and caregivers through individualized spiritual assessments and compassionate presence
- Participate actively in hospice team care conferences, admissions, and Interdisciplinary Group (IDG) meetings
- Conduct timely spiritual assessments and support patients in exploring essential life questions
- Assist with bereavement follow-up care in collaboration with bereavement coordinator
- Develop and maintain relationships with local spiritual leaders
- Provide emotional and spiritual support to hospice team members
- Participate in quality improvement programs and weekend on-call rotations
- Maintain accurate electronic medical documentation of care plans and interventions
Required Qualifications
- Ecclesiastical endorsement by faith group/denomination
- Minimum of two units of Clinical Pastoral Education (CPE) from Accredited ACPE Center
- Master of Divinity degree or equivalency preferred
- One year minimum of hospice care experience preferred
- Demonstrated knowledge of hospice philosophy of care
- Awareness of end-of-life practices across different faith traditions
- Excellent verbal and written communication skills
- Ability to work independently and collaboratively
- Valid driver's license and reliable, insured transportation
- Successful completion of criminal and abuse background checks
