Chaplain, Hospice

Position Overview

Hospice Chaplains serve a sacred role in providing compassionate spiritual support to patients and families during their end-of-life journey, offering deep listening, comfort, and meaningful spiritual care across diverse belief systems.

Key Responsibilities

• Provide comprehensive spiritual care support to patients, family members, and caregivers through individualized spiritual assessments and compassionate presence
• Participate actively in hospice team care conferences, admissions, and Interdisciplinary Group (IDG) meetings
• Conduct timely spiritual assessments and support patients in exploring essential life questions
• Assist with bereavement follow-up care in collaboration with bereavement coordinator
• Develop and maintain relationships with local spiritual leaders
• Provide emotional and spiritual support to hospice team members
• Participate in quality improvement programs and weekend on-call rotations
• Maintain accurate electronic medical documentation of care plans and interventions

Required Qualifications

• Ecclesiastical endorsement by faith group/denomination
• Minimum of two units of Clinical Pastoral Education (CPE) from Accredited ACPE Center
• Master of Divinity degree or equivalency preferred
• One year minimum of hospice care experience preferred
• Demonstrated knowledge of hospice philosophy of care
• Awareness of end-of-life practices across different faith traditions
• Excellent verbal and written communication skills
• Ability to work independently and collaboratively
• Valid driver's license and reliable, insured transportation
• Successful completion of criminal and abuse background checks
