Hospice Chaplain - Pastoral/Spiritual Counselor

Position Overview

AdvisaCare is seeking a compassionate Hospice Chaplain to provide spiritual and pastoral support to patients, families, and caregivers during challenging end-of-life journeys in the Northern Michigan region.

Key Responsibilities

• Conduct comprehensive spiritual and pastoral assessments of patients, families, and caregivers to understand their unique spiritual needs
• Develop individualized spiritual care plans, collaborating with local clergy and addressing patient/family preferences
• Perform home visits and participate in weekly office meetings and interdisciplinary group (IDG) conferences
• Prepare accurate clinical and progress notes in Electronic Medical Records
• Provide bereavement support to staff and families
• Ensure compliance with AdvisaCare Hospice policies, procedures, and Medicare regulations
• Participate in continuous performance improvement programs
• Maintain high levels of customer satisfaction for clients, families, and caregivers

Required Qualifications

• Graduate degree from an accredited seminary or school of theology, or appropriate pastoral ministry certification
• Demonstrated experience working with patients and families experiencing loss
• Strong empathy and ability to adapt to patient's changing status and needs
• Excellent critical thinking skills
• Proficiency in electronic medical record documentation
