Position Overview
AdvisaCare is seeking a compassionate Hospice Chaplain to provide spiritual and pastoral support to patients, families, and caregivers during challenging end-of-life journeys in the Northern Michigan region.
Key Responsibilities
- Conduct comprehensive spiritual and pastoral assessments of patients, families, and caregivers to understand their unique spiritual needs
- Develop individualized spiritual care plans, collaborating with local clergy and addressing patient/family preferences
- Perform home visits and participate in weekly office meetings and interdisciplinary group (IDG) conferences
- Prepare accurate clinical and progress notes in Electronic Medical Records
- Provide bereavement support to staff and families
- Ensure compliance with AdvisaCare Hospice policies, procedures, and Medicare regulations
- Participate in continuous performance improvement programs
- Maintain high levels of customer satisfaction for clients, families, and caregivers
Required Qualifications
- Graduate degree from an accredited seminary or school of theology, or appropriate pastoral ministry certification
- Demonstrated experience working with patients and families experiencing loss
- Strong empathy and ability to adapt to patient's changing status and needs
- Excellent critical thinking skills
- Proficiency in electronic medical record documentation
