Hospice Chaplain - Pastoral/Spiritual Counselor

Position Overview

AdvisaCare is seeking a compassionate Hospice Chaplain to provide spiritual support and pastoral counseling to patients, families, and caregivers during challenging end-of-life journeys in Northern Michigan.

Key Responsibilities

• Conduct comprehensive spiritual and pastoral assessments of patients, families, and caregivers to understand their unique spiritual needs
• Develop individualized spiritual care plans, collaborating with local clergy when desired by patients
• Participate in interdisciplinary group (IDG) conferences and prepare detailed clinical and progress notes
• Provide bereavement support to staff and maintain accurate Electronic Medical Records
• Ensure compliance with AdvisaCare Hospice Policies, Procedures, and Medicare regulations
• Maintain high levels of customer satisfaction through empathetic and adaptive patient care
• Contribute to continuous improvement of Hospice services through critical thinking and performance improvement programs

Required Qualifications

• Graduate degree from an accredited seminary or school of theology, or appropriate pastoral ministry certification
• Demonstrated experience working with patients and families experiencing loss
• Strong empathetic approach to patient and caregiver interactions
• Ability to adapt to changing patient status and needs
• Excellent critical thinking and communication skills
• Willingness to participate in home visits and weekly office meetings
